The Choices team can assist you in understanding and preparing an Advance Health Care Directive and/or a POLST (Physician Orders for Life-Sustaining Treatment) form. These are legal documents that ensure your family, physicians, and hospital staff know what your wishes are should you be unable to speak for yourself. Having these decisions made prior to a medical crisis can ease the burden on those who will be called upon to make healthcare decisions for you.
